The main manifestations of miscarriage are vaginal bleeding and abdominal pain. In early miscarriage, vaginal bleeding often occurs first, followed by lower abdominal pain. If the miscarriage occurs before 8 weeks of pregnancy, since the villi have not yet matured and their connection with the uterine decidua is not firm, the entire blastocyst and villi are often completely detached from the uterine wall and expelled. During expulsion, the embryo is wrapped in decidua and blood clots, resulting in relatively little bleeding. If the miscarriage occurs between 8 and 12 weeks of pregnancy, as the villi have developed vigorously and deeply embedded in the decidua, their connection with the decidua is more firm. The placenta often cannot be completely detached and expelled along with the fetus, so it is common to have only the fetus expelled alone, or the fetus and part of the placenta expelled. This leaves some tissue remaining in the uterine cavity, obstructing uterine contraction and affecting the closure of blood sinuses, leading to more significant blood loss. After 12 weeks of pregnancy, the placenta is fully formed, and during miscarriage, it is expelled intact after the fetus is delivered. Therefore, the process of late miscarriage is similar to that of preterm labor and full-term delivery, starting with intermittent uterine contractions, followed by cervical dilation, and then the delivery of the fetus and placenta.
