Features: In recent years, there have been many significant advancements in hernia surgery. It has been a great honor to serve as the editor of the 5th edition of (GHernia), and I am truly excited. Now, the book has been renamed 'Nyhus Condun's Hernia. Although the major changes in surgery in recent years have primarily occurred in the treatment of inguinal hernias, many new treatment principles have also emerged in the management of incisional hernias, umbilical hernias, diaphragmatic hernias, and other rare types of hernias. During the first half of the 20th century, the Rassini operation and its derivatives, such as the Marcy, McVay, Halstead, Andrews, Ferguson, and Shouldice operations, were widely used in hernia repair surgeries. These procedures involved suturing the hernia defect under tension. The results of these tension-free repairs were acceptable at the time, but they were often associated with severe postoperative pain and higher recurrence rates than surgeons had estimated. In the 5th edition of (HNyhus. London's Hernia), the key treatment principles summarized and some sections differing from previous editions were only recently proposed. The shift in surgeons' perspectives is based on four principles that have influenced their surgical techniques: ① the widely accepted 'tension-free' principle; ② the routine use of repair materials; ③ the recognition that the preperitoneal space can be used for hernia repair; and ④ the use of laparoscopic surgery for therapeutic purposes. 'For surgeons, hernias have always been an intriguing topic, and this is even more evident today. The inguinal hernia region resembles a mysterious, impregnable fortress, constantly under attack. Discussions about sliding inguinal hernias often spark controversy. Many previously debated areas have recently been revisited for further discussion and research, with new knowledge emerging seemingly every day. Please note that this introduction is from the 5th edition, but it was originally written for the preface of the 1st edition of (Ilernia) 37 years ago. As new knowledge has been introduced into this field and applied, new and ongoing debates have also emerged. The contributions of this edition are to propose new paradigms for hernia surgery. Differing opinions drive the pursuit of knowledge, which in turn raises and clarifies many important questions. The new knowledge that emerges during this process can be directly applied to patients. Many chapters have been revised and updated with recently published content. The book is comprehensive and rich in information, but it lacks very recent research findings. Some materials, although thorough, with randomized and prospective studies and trials, were not included due to the specificity of the arguments. However, we have adopted materials that, while not widely representative in today's literature, present substantive arguments and are presented in a scientific manner. As in previous editions, we have strived to gather the most recent and appropriate information to help surgeons effectively and efficiently treat patients anywhere.
