Constantly losing semen during sleep and unable to hold it in, what can be done?

Patient's question:

I always have nocturnal emissions and can't hold it in. Sometimes when I wake up in the morning, I find it's all over the place. It's been like this for a long time. Help me.

Doctor's answer:

Ejaculation involuntary, also known as "slippery leakage," refers to the condition of nocturnal emission without dreams, or even the spontaneous leakage of semen during wakefulness.
Ejaculation involuntary is a type of nocturnal emission and represents a more severe stage of it. Before the Sui and Tang dynasties, physicians believed that nocturnal emission was caused by consumptive diseases. It was referred to by various names such as "dreamed ejaculation," "dream leakage," and "semen leakage." In the Treatise on the Causes and Symptoms of Diseases (Zhubing Yuandehlun) by Qiao Yuanyang of the Sui dynasty, it was stated: "Deficiency of kidney qi leads to the inability to store essence, resulting in semen leakage." This theory attributed slippery leakage to the weakness of the kidney and the failure of the essence gate to hold the semen. By the Yuan dynasty, Zhu Danxi, while inheriting the earlier belief in deficiency, also proposed that ejaculation involuntary was related to damp-heat accumulation in the lower body that disturbs the essence chamber. In the Danzhi Xinfa (Heart Method of Danxi) section on nocturnal emission, he wrote: "Slippery leakage is primarily caused by damp-heat. Huangbai and Zhimo clear heat, while Oyster Powder and Clam Powder dry dampness." This highlighted the aspect of excess in ejaculation involuntary. Clinically, it is more common for ejaculation involuntary to be caused by kidney deficiency failing to grasp essence. The primary treatment for ejaculation involuntary involves warming the yang, tonifying the kidneys, and consolidating essence to stop leakage.
