Why does sexual intercourse not bring pleasure

Patient's question:

According to what I read online and in books, sex seems to be very enjoyable, but after a few sexual encounters, I feel a bit lost. During the process, I don't feel any pleasure, only during ejaculation do I get a sense of it, and it doesn't feel as comfortable as masturbation. What's going on? Is it related to masturbation?

Doctor's answer:

Chronic inflammation and congestion of the prostate, along with atrophy of the gland, can persistently stimulate the ejaculatory duct, leading to local hypersensitivity to sexual impulses and stimuli, making it easier to experience premature ejaculation or rapid ejaculation. Over time, this can cause fatigue and insensitivity in the neural reflex system of the ejaculatory duct and ejaculation, potentially lowering the patient's sexual arousal and ejaculation threshold. Prostate inflammation can excessively stimulate internal nerve tissues, reducing the prostate's excitability. Prolonged congestion and edema of the prostate can affect ejaculation ability and time. Dysregulation of the autonomic nervous system and abnormal sexual psychology can significantly diminish or completely eliminate sexual pleasure. Additionally, chronic inflammation can prevent the higher central nervous system from getting adequate rest and functional adjustment, leading to insomnia, frequent dreams, weakened constitution, and physical exhaustion, all of which are important reasons for the loss of sexual pleasure in patients.
