How to care after varicocele surgery?

Patient's question:

Medical Description: I am 22 years old, male, and had surgery for left varicocele high vein ligation two years ago. So far, the varicocele has recovered, but strangely, the epididymis of my left testicle is still hard. When I press it with my fingers, it causes pain. Usually, if I rest for a few days, it becomes slightly softer. However, if I have a longer erection, drink some alcohol, or engage in more intense exercise, the epididymis becomes hard again... Is this normal? Could it still be bruised inside?
History of Treatment and Effects: During a follow-up after surgery, I mentioned this condition to the doctor, but he said it was normal. So I didn’t pay much attention, not expecting recovery to be this slow after so long. There was no further treatment during this period.
What I’m Thinking:

Doctor's answer:

Varicocele, due to blood stasis in the spermatic veins, can cause an increase in scrotal temperature, with the testicular temperature rising by 1℃ to 2℃. Prolonged high temperatures can impair sperm production in the testes, leading to azoospermia and oligozoospermia. The epididymis also does not favor sperm maturation. This condition is more common in young and middle-aged adults. The left-side varicocele has a higher incidence rate. Those with mild or no symptoms can wear elastic pants or use a scrotal support band.
For men with severe symptoms and varicocele accompanied by abnormal sperm, surgical treatment is recommended. The surgical procedure involves high ligation of the spermatic veins. I’m not sure about your specific condition, but it’s recommended to visit a hospital for diagnosis and targeted treatment.
