Why does varicocele cause testicular pain?

Patient's question:

It seems like there are things like worms in my testicles, and sometimes it's very painful. Should I have surgery immediately? Sperm motility is also low. What should I do?

Doctor's answer:

Hello, symptoms of varicocele:
1. The patient may be completely asymptomatic and only discovered during a physical examination.
2. A sense of heaviness, fullness, or pain in the affected scrotum or testicle, swelling of the scrotum, the affected scrotum and testicle being lower than the healthy side when standing, and dilated, tortuous veins visible on the scrotal surface. A soft, worm-like mass can be felt upon palpation, and symptoms may alleviate or disappear when lying down.
3. The patient may experience neurotic symptoms such as headaches, fatigue, or hyperesthesia. Some patients may have sexual dysfunction.
Treatment
Mild varicocele without symptoms does not require treatment.
Non-surgical treatment: For mild varicocele or those accompanied by neurotic symptoms, measures such as scrotal support and cold compresses can be used.
Surgical treatment: For severe varicocele, a history of sperm count below 20 million three times consecutively, or testicular atrophy; or when the dilated veins disappear when lying down, high ligation of the internal spermatic vein can be performed.
Surgery should be considered, and specific treatment measures should be taken under the guidance of a doctor.
