Sometimes the testicles feel pain when sleeping, what is the reason?

Patient's question:

I sometimes feel a pain in my testicles~ a sense of swelling and pain~ Please ask if it's okay~

Doctor's answer:

Testicular pain can be broadly categorized into two types: acute persistent pain and chronic frequent pain.
Acute Pain
Acute pain is more commonly associated with orchitis and injury.
Orchitis, in addition to hematogenous infection, is more often caused by bacteria retrograding from the urethra to the epididymis and testes, leading to epididymitis and orchitis. Clinically, this presents as swelling and pain in the epididymis and testes.
Chronic Pain
Chronic pain may persist for a long time, even in mild cases.
The pain is relatively mild, diffuse, and radiating, making it difficult to determine the exact location of the inflammation.
The pain in the testes does not necessarily correlate with the severity of the inflammation. Some individuals with high nerve sensitivity may experience severe pain even with mild inflammation, while others may perceive it as less intense.
