Erectile dysfunction without sexual desire: What could be the reason?

Patient's question:

Hello Doctor!
I am 35 years old. Recently, I have been experiencing some difficulty in urination, a feeling of incomplete emptying of the bladder, slight dampness in the scrotum, some lower back pain, poor libido, weak and short erections, and a slightly smaller left testicle compared to before. Could this be a problem with the prostate? What medication should I take? Thank you for your reply!

Doctor's answer:

Hello!
Prostatitis is one of the most common diseases among middle-aged men, with an onset age range of 15-55 years. It can be categorized into: acute prostatitis and chronic prostatitis, the latter of which can be further divided into: bacterial prostatitis, non-bacterial prostatitis, and prostatodynia. Prostatitis exhibits a wide range of symptoms with significant individual variations. Currently, the symptoms of prostatitis are collectively referred to as prostatitis syndrome, which can be broadly classified into the following four categories:
1. Urethral symptoms: Frequent urination or a constant urge to urinate, a feeling of incomplete urination, mild urinary pain, pain at the urethral meatus, and occasional leakage of white, viscous fluid from the urethra.
2. Local symptoms: Lower abdominal heaviness or dull pain; pain or discomfort in the genital area or testicles, a damp sensation in the perineum, pain in the inguinal region, lower back pain, and fatigue.
3. Sexual dysfunction: Premature ejaculation, erectile dysfunction, and reduced libido.
4. Mental symptoms: Irritability, excessive worry, frequent dreams, insomnia, and palpitations.
Among these four categories of symptoms, not all patients will experience them. Depending on the severity of the condition and the treatment approach, there can be significant clinical differences.
The treatment of prostatitis is a research topic in urology. Monotherapy with a single drug is often not highly effective. Currently, a comprehensive treatment approach is primarily adopted. After a period of systematic treatment, symptoms are alleviated in most patients, and some may even be cured. In China, there is a recent breakthrough in the treatment of prostatitis called "field effect ablation," which has shown promising results in treating prostatitis. Before undergoing any treatment, it is best not to self-medicate, as incorrect medication can have counterproductive effects on the disease.
If you have any further questions, please feel free to ask. Wishing you a speedy recovery!
