Penile plaques are caused by an unknown infectious process of the tunica albuginea or autoimmune inflammation leading to the local elastic tissue being replaced by hyalinization and fibrotic scarring, which can even result in fibrosis and ossification. The main symptoms are fibrosis of the penile connective tissue, with one or more hard plaques and cords on the dorsal side of the penis. During erection, the penis may bend upward or to the left or right, accompanied by slight pain. If the bend is significant, it can interfere with sexual activity. Early treatment can involve local hormonal anesthesia, which is quite effective. If the anesthetic medication is not effective, radiotherapy can be considered. If the erection is severely affected or significant pain recurs, surgical removal of the scar may be necessary.
