How to treat orchitis

Patient's question:

Pain and swelling near the genital area, lying down can relieve it, but standing for a long time doesn't help, and it also affects sexual life. How should orchitis be treated?

Doctor's answer:

Orchitis, besides hematogenous transmission, is more commonly caused by bacteria retrograding from the urethra to the epididymis and testes, leading to epididymitis and orchitis. It is also seen after sexual activity, varicocele, prostatitis, ureteral stones, etc.
Treatment for orchitis generally involves bed rest with scrotal support to prevent sexual activity. Antibiotics are commonly prescribed: penicillin 800,000 units per dose, 2-4 times daily, administered via intramuscular injection; or penicillin 4-6 million units per dose, 2 times daily, via intravenous infusion. Those allergic to penicillin may be prescribed erythromycin 0.9-1.2g per day, infused intravenously with 5% glucose.
