How is it that one testicle is bigger than the other?

Patient's question:

I recently discovered that one of my testicles is larger than the other. This symptom started about a week ago, with pain and slight swelling in the testicle, which feels a bit hard. Sometimes it can be very painful, especially when touched. My penis is small, and this has been the case for 7 years. Additionally, I have a habit of excessive masturbation, about three to four times a week. What could be the reason for one testicle being larger than the other?

Doctor's answer:

Under normal circumstances, the two testicles are not the same size, but they should be relatively similar. If one testicle is significantly larger than the other, that is not normal. There are two possible reasons: one is that the larger testicle may have symptoms of hydrocele, in which case an ultrasound examination is needed to diagnose it. Surgical treatment may be necessary if necessary. The other possibility is that the smaller testicle has atrophied, and an ultrasound examination is required to identify this. This condition needs to be treated, and after treatment, there will be no adverse effects.
