What is the reason for abdominal pain after medication abortion?

Patient's question:

I had a medication abortion on November 15th. The medication didn't come out on the day I took it, but it did come out on the second night after I went home.
At that time, the abdominal pain was very severe, but it got better later. However, starting last week, I've been experiencing abdominal pain every day from 4 PM to 7:30 PM. The pain is accompanied by leg soreness and hip pain on both sides.
I had an ultrasound done twice, and both said it was completely cleaned out, but there was still something coming out of the vagina. It smells foul.
Could you please tell me what's going on?
Is the abdominal pain happening at almost the same time every day? Thank you.

Doctor's answer:

Pain is normal, it usually happens, but if the pain is severe, it's better to go to the hospital and ask a doctor to see what the problem is. Don't keep putting it off, because delaying it can lead to complications. If it's not completely cleaned out, it could cause trouble. It's recommended to go to the hospital for a check-up. There are three reasons for abdominal pain after a medication abortion:
1.Incomplete abortion: Incomplete abortion is the main cause of pain after medication abortion. It can generally be diagnosed through a gynecological examination and ultrasound.
2.Infection after medication abortion: If the ultrasound shows no residual tissue in the uterus, it's necessary to consider an infection in the uterus, as the blood from the abortion provides a good breeding ground for bacteria. If the abdominal pain is severe after medication abortion, the uterus may be tender during a gynecological examination, and symptoms like fever may also occur.
3.Uterine hematoma: This often occurs within a few hours after medication abortion. The patient may experience severe abdominal pain, like a knife-cutting sensation, with a small amount of bloody discharge from the vagina. A gynecological examination may reveal a significantly enlarged, soft, and tender uterus. Uterine hematoma is relatively rare, but women with an excessively tilted uterus or very tight cervical openings are more prone to experiencing abdominal pain after medication abortion.
