How to care for perineal episiotomy after childbirth

Patient's question:

I have an itchy lower body, and my underwear is always yellow. What's going on? How should I take care of episiotomy after childbirth?

Doctor's answer:

The perineum refers to the soft tissue between the anus and the vaginal opening. During childbirth, due to the pressure exerted by the fetus on the perineum and the medical procedures performed on it, the perineum often becomes congested and swollen. Some may also suffer from varying degrees of lacerations or have episiotomies performed for the baby's delivery. The wound in the perineum is located near the anus and urethral opening, and after childbirth, the vagina continuously discharges lochia. As a result, it is easily contaminated by feces, urine, and lochia. Additionally, the body's weakened state after childbirth makes it easy to develop puerperal infection if hygiene is not maintained. Therefore, proper perineal care after childbirth is crucial for the's early recovery.
First, it is essential to maintain local cleanliness. Sanitary supplies and underwear should be changed and washed frequently. After washing, they should be exposed to sunlight to achieve sterilization. At least 2-3 warm water rinses with a dedicated cleaning basin should be performed on the vaginal area daily after childbirth. If there are lacerations or episiotomy wounds in the perineum, a high-manganese potassium permanganate solution diluted 5000 times should be used for 2-3 rinses daily, and an additional rinse should be done after each bowel movement. For swollen perineums, a warm compress with a 50% magnesium sulfate solution can be applied. When lying down, the patient should position themselves on the opposite side of the wound to prevent lochia from flowing toward it and increasing the risk of infection.
After the stitches are removed, the wound's internal healing is not yet firm, so excessive movement and strenuous exercise should be avoided. Avoid squatting or straining during bowel movements to prevent the wound from reopening. Additionally, attention should be paid to dietary balance to prevent constipation.
