Pathologically, it is observed that the uterine muscle layer becomes thickened, with islands of endometrial glands and stroma interspersed within the muscular wall, accompanied by smooth muscle hyperplasia. Clinically, it primarily manifests as dysmenorrhea (painful menstruation), increased menstrual flow, and uterine enlargement.It is generally believed that trauma to the uterine wall during multiple pregnancies and deliveries, as well as chronic endometritis, may be the main causes of this condition. Additionally, since the submucosal layer is absent beneath the basal membrane of the endometrium, and adenomyosis is often associated with uterine fibroids and endometrial hyperplasia, some researchers suggest that the invasion of basal endometrium into the muscle layer may be related to high estrogen stimulation.
