1. Intense sexual activity: First, consider whether the intensity or violence of the activity during intercourse led to strong contractions of the cremaster muscle, causing the testicle with an excessively long spermatic cord to twist and resulting in severe pain.
2. Testicular injury: Testicular injury, local swelling, and bruising can also cause testicular pain after sexual activity.
3. Testicular torsion: Testicular torsion is one of the common acute conditions in the scrotum. It occurs in 65% of cases in individuals aged 12–19. Several hours before the onset, there may have been intense physical activity or external force applied to the testicle. It suddenly causes severe testicular pain during sleep or rest.
4. Testicular inflammation: In addition to hematogenous infection, testicular inflammation is more commonly caused by bacteria traveling retrograde from the urethra to the epididymis and testicle, leading to epididymitis and orchitis. Clinically, it presents with swelling of the epididymis and testicle along with testicular pain.
