The dominant antigen inherited by the fetus from its parents is one that the mother lacks. This antigen invades the mother's body, producing immune antibodies that pass through the placental villi into the fetal blood circulation, clumping with fetal red blood cells and causing their destruction, leading to hemolysis, anemia, edema, hepatosplenomegaly, and progressive severe jaundice shortly after birth, which may even result in bilirubin encephalopathy.

In severe cases, timely treatment after birth can also lead to rapid improvement, and the child will grow up to be indistinguishable from normal children. Early bilirubin encephalopathy may still have a chance of recovery after exchange transfusion, while late-stage cases often have sequelae. In cases of generalized edema, even with active treatment, the chances of success are relatively low.
