What are the specific manifestations of severe fatty liver?

Patient's question:

I was naturally on the heavier side, and I guess there might be some generational genetic factors involved. On top of that, I don't move much after work and spend most of my days sitting in a chair. Last year's health check-up revealed mild fatty liver, and this year, it's become moderate fatty liver. Sigh, the doctor said if I don't start dieting soon, it could become severe next year. I'd like to ask what the symptoms of severe fatty liver are like?

Doctor's answer:

Severe fatty liver generally presents with symptoms such as: fatigue in the limbs, right shoulder and back pain with swelling, feeling tired for no reason, dizziness, bitter taste in the mouth, dry mouth, bad breath, poor appetite, irritability, abdominal pain after meals, nausea, feeling full after eating a small amount, discomfort in the liver area, alternating constipation and diarrhea. In some cases of severe fatty liver, liver function tests may be abnormal with elevated transaminases. Shortness of breath may occur after climbing a few flights of stairs or walking a few extra steps. Severe fatty liver can also lead to mild hyperbilirubinemia, bilirubinuria, elevated urinary bilirubin, and an inverted albumin-to-globulin ratio. The symptoms of severe fatty liver are more pronounced, and patients may experience ascites and lower limb edema, as well as electrolyte imbalances such as hyponatremia and hypokalemia. The daily treatment for fatty liver primarily involves symptomatic management. Patients can take oral medications and supplement with vitamins.
