Is varicocele surgery necessary on the left side?

Patient's question:

After six months of marriage, I couldn't get pregnant, so I went to the hospital to check my semen. The sperm motility rate was 50.51%, which is abnormal. The results were as follows: Grade A: 16.27%, Grade B: 22.37%, Grade C: 11.86%, and Grade D: 49.49%.
Later, the doctor recommended I have a color Doppler ultrasound, which revealed varicocele in the left spermatic cord, measuring about 2.2 mm. The Valsalva test showed venous blood reflux. The right side was normal. Currently, I have mild pain in my left scrotum. The doctor suggested surgery, but I'm a bit scared. The doctor said that even after the surgery, there's no guarantee of pregnancy. My chlamydia and mycoplasma tests were also normal.

Doctor's answer:

Asymptomatic mild varicocele does not require treatment.
Non-surgical treatment: For mild varicocele or those accompanied by neurasthenia, measures such as scrotal support and cold compresses can be used.
Surgical treatment: For severe varicocele, continuous sperm count below 20 million three times in a row, or testicular atrophy; if the varicose veins disappear when lying flat, high ligation of the internal spermatic vein can be performed.
