Patient's question:
Recently, I felt uncomfortable during intimacy with my wife. Usually, everything is normal, and I have never had any diseases before. But recently, I've been experiencing issues. Additionally, I've noticed frequent urination, urgency, and pain during urination. When I visited a doctor, they diagnosed me with prostatitis.Doctor's answer:
General treatment should involve bed rest for 3-4 days, increased fluid intake, and avoiding alcohol and spicy foods. Warm Sitz baths or perineal heat applications can be performed, and bowel movements should be kept regular. Sexual activity is prohibited.Antibiotic treatment:
1) When the patient exhibits obvious systemic symptoms, high fever, and significantly elevated white blood cell counts, intravenous administration should be used. After 1 week, oral medication should be switched to continue for 1 month.
2) A broad-spectrum intravenous antibiotic capable of diffusing into the prostate and acting rapidly should be selected to quickly control symptoms and prevent the progression to chronic prostatitis. Once bacterial culture results are available, a sensitive antibiotic should be chosen.
Symptomatic treatment: If urinary difficulties or retention occur, temporary suprapubic cystostomy should be performed for urine drainage, or a fine, soft silicone catheter can be left in place.
Surgical treatment: If acute prostatitis has developed into a prostatic abscess, drainage surgery should be performed via the rectum or perineum. If the abscess is confined to the prostate, a urethroscope can be used for prostatic puncture and abscess drainage, followed by the injection of a broad-spectrum antibiotic.
