With ejaculation action but no semen released

Patient's question:

During sexual activity, there is an ejaculation action, but nothing comes out.

Doctor's answer:

Hello, the treatment for azoospermia primarily depends on the underlying cause of the condition.
① For obstructive azoospermia, various surgical procedures can be performed depending on the specific situation, such as vasovasostomy, epididymovasostomy, artificial varicocele creation, epididymal sperm retrieval, and transurethral incision.
② For non-obstructive azoospermia, if it is due to endocrine abnormalities, such as impaired secretion of pituitary gonadotropins, treatment with gonadotropin-releasing hormone (GnRH) and gonadotropins, as well as testosterone supplementation, may be effective. If the cause is a pituitary tumor, surgical removal is recommended. For pituitary microadenomas or idiopathic hyperprolactinemia, bromocriptine can be used. In cases of primary testicular failure accompanied by varicocele, varicocelectomy should be performed.
If a man has congenital absence of testicles, severe testicular hypoplasia, secondary testicular atrophy, or azoospermia confirmed by testicular biopsy, it is generally not possible to restore spermatogenesis through treatment. In such cases, infertile couples are advised to use donor sperm for artificial insemination or consider adoption.
