What medication should be used for orchitis?

Patient's question:

Since the day before yesterday, I have been experiencing pain in my left testicle that radiates to this groin area, with a noticeable dragging sensation. Lying down doesn’t cause any discomfort (but I have to work during the day), so I suspect it might be orchitis. Could you recommend what medication I should take? Thank you in advance to this lifesaver.
First follow-up question: Thank you, Doctor Nai Ren Shi, for your advice. The night before yesterday, I saw a doctor who diagnosed me with epididymitis. I have been taking Ofloxacin (, which is a brand name for Ofloxacin dispersible tablets) and have taken it three times so far. Now I feel much better—the pain and dragging sensation have significantly subsided. When I touched it, I found that the epididymis is still about the size of a peanut.

Doctor's answer:

Hello:
During treatment, it is recommended that the patient rest in bed with the scrotum elevated and apply local heat. If the scrotal skin is significantly swollen, a 50% magnesium sulfate solution should be used for to facilitate the resolution of inflammation. For severe pain that does not respond well to analgesics, a 1% procaine block of the affected spermatic cord with 10 mL of procaine can be administered. Systemic medication should include broad-spectrum or gram-negative bacteria-sensitive antibiotics, such as penicillin, gentamicin, and various cephalosporins.
Traditional Chinese medicine primarily uses herbs that harmonize the body, promote pus discharge, clear heat, and reduce dampness. Medicinal decoctions or prepared formulations based on the basic formula of Longdan Xiegan Tang can be used. A topical application of herbs like Jinhuang ointment, which clears heat and detoxifies, can also be applied to the affected area. If a testicular abscess has formed, consider draining the pus by aspiration or incision. If the testicle is completely destroyed by pus, the affected testicle may be removed.
Acute bacterial orchitis is severe once it occurs and should not be ignored or delayed in treatment to avoid serious consequences. Strengthening personal hygiene and promptly controlling infections in the body are effective measures to prevent this condition.
