Patient's question:
Medical Description: I noticed that my boyfriend's semen is very thin and has a strong fishy smell. If he hasn't had sex for a while, the first time his semen is slightly creamy and a bit thick. If he has sex several times a day or daily, I find his semen is watery and very fishy, which is completely different from my previous boyfriend's. Is this normal? My boyfriend says the fishy smell indicates good semen quality—is that true?History of Treatment and Effects: No treatment history
What Help Do You Want: I want to know what's going on. I also noticed that his erection isn't very firm, and he needs my help to penetrate each time. Is there a problem? Could it affect fertility? Thank you
Doctor's answer:
Under normal circumstances, semen expelled from the body is a grayish-white, viscous liquid. Due to the action of coagulase in seminal vesicle secretions, it can quickly solidify into a gel-like state. This coagulation helps prevent semen from leaking out of the vagina after sexual intercourse. However, this coagulated state is temporary and begins to liquefy within about 15 minutes, turning into a thin, flowing liquid. This is generally a normal phenomenon. However, if the semen remains very thin and scarce after liquefaction, it warrants attention, as it may be caused by inflammatory conditions of the urinary and reproductive systems, such as orchitis, epididymitis, seminal vesiculitis, prostatitis, or urethritis. Patients with such conditions account for the highest proportion among those with oligospermia and should seek timely medical examination and treatment.
