What should be done about bilateral varicocele?

Patient's question:

Hello: I am a 22-year-old patient with a history of bilateral varicose veins and prostatitis. The prostatitis has improved, but the left side is still at a degree of 2 or higher, with yellow semen that is very thin and watery. The results of the last check-up showed a sperm count of 7.5 million and a motility rate of 35%. Since taking the medication "Dongweili" for a week, the semen has become much thicker. Another check-up revealed a sperm count of 9.6 million and a motility rate of 67%, which has reached normal levels. However, there is still some discomfort in the scrotum sometimes. Can I consider not having surgery? Will continuing to take this medication be effective? If it is still necessary, for bilateral varicose veins, which type of surgery would be most suitable (without considering cost).

Doctor's answer:

Hello: It is essential to actively treat it to avoid infertility! Varicocele has little impact on overall health, and most cases can resolve on their own after marriage. Once diagnosed, it is important to rest, maintain a balance between activity and rest, and actively participate in beneficial cultural and recreational activities.
For patients with more severe symptoms that may affect fertility, surgical treatment should be considered, specifically high ligation of the spermatic vein. Those with mild symptoms may not require any treatment and can use a scrotal support band to elevate and protect the scrotum.
Local steam bathing may provide some therapeutic benefits. The medicinal herbs include 30 grams each of Astragalus, Sambucus, and Salvia miltiorrhiza, 10 grams each of Foeniculum, Notopterygium, and Carthamus, decocted in boiling water and used to steam the affected area. This should be done twice daily for 30 minutes each time. One decoction can be used for 2–3 days.
