Features: Erectile dysfunction (ED) is a common condition among middle-aged and elderly men, with an incidence rate of approximately 50%. It also occurs in younger individuals. Severe cases can significantly impact quality of life and even lead to family breakdown, drawing increasing societal attention. However, due to prolonged influence from feudal consciousness, the lack of effective medications, and false advertising and deceptive practices by unlicensed practitioners, this condition has been shrouded in mystery. Many patients fail to receive timely and effective treatment for extended periods, resulting in immense psychological burdens, which is deeply concerning. Scientists and researchers must address this social issue by promoting awareness and disseminating accurate scientific knowledge to educate the public about the normal physiological functions and pathological processes of the reproductive system. When symptoms arise, prompt diagnosis and treatment are essential. Patients should confidently seek care at specialized, reputable hospitals rather than relying on false advertisements or the misleading claims of unlicensed practitioners, which can delay treatment and harm both themselves and their families.
With advancements in science and technology, particularly the introduction of effective medications such as Pfizer's "Viagra" (sildenafil), the treatment of ED has seen significant breakthroughs, which is a welcome development. We hope that more and more effective drugs will be developed in the future. However, we must also recognize that no treatment is universally effective, with an efficacy rate typically ranging from 70% to 80%. This means that 20% to 30% of patients may still require alternative methods, such as intravesical medication, intracavernous drug injection, vacuum negative pressure devices, or constriction rings. Even in organic cases, 10% or more may need surgical intervention. Although surgical treatment is invasive, it can restore function in patients who do not respond to other treatments.
This book provides a systematic and detailed introduction to the pathogenesis of ED and its surgical treatment, with clear step-by-step procedures. It aims to enable urologists and andrologists with surgical expertise to perform this work after reading it. Of course, it is crucial to strictly adhere to surgical indications and contraindications, emphasize surgical technique, ensure proper postoperative care, and strive to improve surgical quality to promote the development of andrology in China and benefit humanity.
