Clinically, especially when caused by increased intracranial pressure, the head undergoes progressive abnormal enlargement out of proportion to overall development. The forehead protrudes forward, the top of the orbits is compressed downward, both eyes gaze downward, and the eyes turn downward, causing the upper sclera to show white. The anterior fontanelle is enlarged and tense, other fontanelles may also be enlarged, cranial sutures are separated, and scalp veins are dilated.
