What are the reasons why a condom cannot be inserted?

I and my partner have always used condoms during sexual activity in the past, but recently I've found that if I don't use a condom, my penis cannot fully insert, and it can only go in halfway. Also, the foreskin on both sides of my penis feels a bit painful when it's stretched. What could be the reason for this?? Is there any way to solve this??
