Hello! Most cases are caused by seminal vesiculitis. Due to the thin wall of the seminal vesicles, if inflammation and congestion occur, or if lesions such as stones appear, the blood-vessel-rich wall of the vesicles can easily bleed. The secretion of the seminal vesicles is one of the main components of semen. When the secretion is stained with blood due to bleeding, and if it is expelled from the body along with semen, hematospermia occurs. Chronic prostatitis can also cause hematospermia, with a mechanism similar to seminal vesiculitis. Since prostatic fluid is another important component of semen, inflammation and congestion of the prostate lead to exudation; frequent sexual activity can cause capillary rupture; and the forceful contraction and relaxation of the prostate during ejaculation can also cause the prostatic fluid to be mixed with blood, thereby staining the semen with blood. Similar to chronic prostatitis, benign prostatic hyperplasia can also lead to hematospermia. It is worth noting that hematospermia caused by chronic prostatitis or seminal vesiculitis is often related to excessive sexual activity or rough actions.
