How to deal with the tortuous left spermatic vein?

Patient's question:

Today morning I had surgery for varicocele, now it feels much worse than before. Lying down is extremely painful, and my left testicle seems to be swollen. What's going on?

Doctor's answer:

The pampiniform plexus of spermatic veins extends, tortuates, and shortens, which is called varicocele. It is more common in young people, with a recurrence rate of 16-25 years old, accounting for approximately 15% of the incidence, and about 99% occur on the left side, with bilateral cases accounting for about 1%.
Treatment for varicocele: Mild varicocele without symptoms does not require treatment. Non-surgical treatment: For mild varicocele or those accompanied by neurasthenia, report scrotal swelling, cold compresses, etc. Surgical treatment: For more severe varicocele, a history of sperm count below 20 million for three consecutive times, or testicular atrophy; if the dilated veins disappear when lying flat, the spermatic vein (high ligation of the spermatic vein) can be performed.
