Bilateral varicocele, how severe is it?

Patient's question:

Left varicocele during normal breathing 1.7mm, holding breath 2.0mm; Right during normal breathing 2.8mm, holding breath 2.1mm. How many degrees is this? It has been a bit painful recently.

General varicocele grading includes:
I degree: The varicose veins are not palpable upon examination, but can be felt when the patient holds their breath to increase intra-abdominal pressure. Contrast imaging of the spermatic vein shows contrast flowing back in the spermatic vein for a length of 5 cm.
II degree: The varicose veins can be palpated upon examination.
III degree: The scrotum is enlarged, and varicose veins can be seen on the surface of the scrotum with the naked eye.
