Is the color Doppler ultrasound report of varicocele serious?

Patient's question:

Two months ago, after drinking beer, I experienced pain in the groin area. A prostate fluid test revealed inflammation, and I have been taking medication ever since. My urination is still not completely normal, though it has improved slightly, but the pain has not decreased. On the 18th, I had a color Doppler ultrasound examination of the prostate, which showed uneven echoes and no obvious abnormalities. The bilateral spermatic veins are gradually dilating; the left one has a maximum diameter of about 2.3 cm, and the right one has a maximum diameter of about 2.5 cm. The doctor recommended surgery. Did the unit for the vein dilation test get written incorrectly—is it cm or mm? Or am I particularly severe? The pain is still persistent now. How should I be treated? Is surgery still necessary?

Doctor's answer:

Pain and discomfort in both groin areas are likely caused by varicocele. Prostatitis also requires systematic treatment. Based on your condition, it is recommended to first undergo systematic treatment for prostatitis and wait until it is completely healed before performing surgery to treat varicocele. The ultrasound report should have a typo; it should be mm. If you experience any discomfort symptoms, varicocele will need surgical treatment. The condition of varicocele tends to recur, requiring long-term medication.
