Cysts originating from the paramesonephric duct are typically located in the midline posterior to the prostate, while cysts originating from the mesonephric duct are situated on both sides. These cysts are not actually derived from the prostate and are often adherent to the posterior wall of the bladder. Their size can often grow very large, compressing the bladder neck to cause urinary difficulties; compressing the rectum to lead to a sense of rectal fullness and defecation difficulties. Congenital prostatic cysts are often associated with other congenital diseases such as hypospadias, cryptorchidism, and renal agenesis. Acquired cysts are caused by the hardening of the prostatic stroma, leading to incomplete or intermittent obstruction of the acini.
