What is the cause of swelling in one testicle?

Patient's question:

I am 17 years old this year, and it has been a year since I had surgery for varicocele. Before the surgery, my testicles were left larger than right, and an ultrasound was done which said it was normal. But now it seems there is fluid inside the left testicle, and it doesn't hurt or itch—it's just that the left testicle is larger than the right one. Is this normal?

Doctor's answer:

Experts suggest that left testicular swelling may be caused by epididymitis. Epididymitis is a common disease among middle-aged and young men, which is caused by pathogenic bacteria such as E. coli, Staphylococcus, or Streptococcus entering the epididymis through the vas deferens. Therefore, this condition is often secondary to posterior urethritis, prostatitis, and seminal vesiculitis. Experts cite that clinical diagnosis is divided into two major categories: acute epididymitis and chronic epididymitis. Chronic epididymitis is more common than acute, and it may cause scrotal pain, a sense of heaviness or fullness, with pain potentially radiating to the lower abdomen and the inner side of the same thigh.
