Chronic orchitis is often caused by incomplete treatment of non-specific acute orchitis. It can also be triggered by infections such as molds, spirochetes, or parasites, such as testicular syphilis. Individuals with a history of testicular trauma may experience recurrent granulomatous orchitis. Local or systemic radioactive phosphorus irradiation can also lead to orchitis, destroying testicular tissue. Pathologically, the testes may appear enlarged or hardened and distended, with the basal membrane of the seminiferous tubules showing hyalinization and degenerative changes, and the disappearance of the seminiferous epithelial cells. Hardening may occur around the seminiferous tubules, or small fibrotic foci may form.
