Rest in bed, support the scrotum, and apply local cold compresses. If the scrotal skin is significantly swollen, use a 50% magnesium sulfate solution for warm compresses to reduce inflammation. For severe pain that does not respond well to analgesics, perform an isolated cord block with 10 mL of 1% procaine on the affected side. Systemic medication should include broad-spectrum or Gram-negative-sensitive antibiotics, such as penicillin, gentamicin, and various cephalosporins. Orchitis tends to recur, requiring long-term medication.
