How should varicocele be treated?

Patient's question:

I have varicocele for a while now, and the symptoms have been getting worse recently. I also feel pain in the groin area, and the testicles feel like there are worms coiled on them. I heard this condition can affect fertility. I've been married for over a year and I'm really worried. Could you please tell me: what is the best treatment for varicocele?

Doctor's answer:

Varicocele can lead to male infertility through various different pathways. It not only disrupts the regeneration of sperm but also causes a decrease in sperm motility. Therefore, patients with severe varicocele still need to consider active treatment. For mild varicocele or those accompanied by neurasthenia, measures such as reporting the scrotum and applying cold compresses are recommended. For more severe varicocele, patients with a sperm count consistently below 20 million for three consecutive times, or those with testicular atrophy; when the varicose veins disappear when lying flat, varicocelectomy (high ligation of the internal spermatic vein) can be performed. The condition of varicocele is prone to recurrence, requiring long-term medication. Moreover, for male patients, it is essential to seek professional and reputable medical treatment immediately and consider treatment based on examination results.
