Ovarian chocolate cyst

After diagnosis, surgery should be performed immediately, as the released cyst fluid can cause pelvic adhesion, infertility, or recurrent spread and implantation of endometrial tissue. For young, childless women, after aspiration and thorough rinsing of the cyst fluid spilled into the pelvic cavity, a cystectomy should be performed while preserving as much normal ovarian tissue as possible. This helps maintain ovarian and endocrine functions and increases the chances of future fertility. For older women with a normal contralateral ovary and an unaffected uterus, to prevent recurrence, the affected adnexa can also be considered for removal.
