Abnormal cervical canal position: Chronic pelvic inflammation or endometriosis may cause the uterus to be excessively retroverted, retroflexed, or antiflexed, making the external cervical os adhere to the anterior fornix. This results in shallowing of the posterior fornix, losing its function as a sperm reservoir, thereby hindering sperm ascent. Additionally, cervical elongation, excessive shortening, or cervical prolapse can also obstruct sperm ascent.
