What are the causes of pelvic infertility?

Patient's question:

Over the past year of marriage, I've always had lower back pain, and I haven't been able to get pregnant. The check revealed pelvic inflammatory disease.

Doctor's answer:

1. Pelvic inflammatory disease (PID) is one of the more common causes of pelvic infertility. The causes of PID are often related to poor hygiene habits of the patient, such as sexual activity during menstruation or within one month after childbirth, or taking a bath after gynecological surgery within one month. In general, PID is primarily caused by poor disinfection during childbirth, miscarriage, or dilation and curettage, neglecting hygiene during the menstrual period or postpartum period, or engaging in sexual activity during menstruation, or using unclean menstrual pads, which can lead to ascending infection.
2. Pelvic effusion can be divided into two types: physiological and pathological. Physiological pelvic effusion does not require treatment, except when it is normal due to physiological characteristics of women. Pathological pelvic effusion is mostly caused by pelvic inflammatory disease or endometriosis. Medical infections caused by poor disinfection during artificial abortion or induction of labor can also lead to pelvic effusion.
3. Lack of hygiene during the menstrual period. During menstruation, the endometrium is shed, blood sinuses in the uterine cavity are open, and blood clots are present, which provide a favorable environment for bacterial growth. If hygiene is not maintained during menstruation, using sanitary pads or toilet paper that do not meet hygiene standards, or engaging in sexual activity, can provide bacteria with the opportunity for retrograde infection, leading to the occurrence of pelvic inflammatory disease.
