Patient's question:
The causes or etiology of ovulatory dysfunctional uterine bleeding (DUB) can be categorized into 4 types:1. Luteal Phase Defect
Ovulation occurs during the menstrual cycle, but the corpus luteum fails to secrete sufficient progesterone or degenerates prematurely. This leads to inadequate secretion of the endometrium.
2. Incomplete Endometrial Desquamation
This is also known as incomplete luteal regression. Due to incomplete atrophy of the corpus luteum, estrogen and progesterone levels do not decline rapidly. The imbalance in hormone levels prevents the endometrium from shedding regularly, resulting in irregular desquamation. This prolongs the bleeding period and increases blood volume, and is also referred to as incomplete luteal regression.
3. Delayed Endometrial Repair
After the endometrium is shed during menstruation, the development of new follicles in the next cycle is delayed or suboptimal. The insufficient secretion of estrogen by these follicles leads to delayed endometrial repair.
Doctor's answer:
The most common treatment for luteal phase defect is to supplement the insufficient progesterone in the body, generally using intramuscular progesterone injections and oral progesterone.Drug-stimulated luteal phase therapy
This primarily uses human chorionic gonadotropin to promote follicular growth. Additionally, for luteal phase defect caused by excessively high prolactin levels in the blood, drugs that reduce prolactin, such as bromocriptine, are required.
