The common causes of oligospermia include: (1) Congenital testicular dysgenesis or cryptorchidism, which can lead to impaired spermatogenesis (2) Varicocele, hydrocele of the tunica vaginalis (3) Inflammation of the reproductive tract (4) Immunosuppression and endocrine disorders (5) Chromosomal abnormalities, especially sex chromosome aberrations, which severely affect sperm density and motility (6) Factors that affect spermatogenesis, such as drugs, insecticides, heavy metals, radiation, smoking, and alcohol intoxication, can all inhibit sperm production. However, there is a subset of patients for whom no cause of sperm reduction can be identified through various examinations, and this is referred to as idiopathic oligospermia.
