Irregular ovulation is caused by what

Patient's question:

Hello doctor, I would like to learn some things. Do women in their 20s have abnormal ovulation, and what causes abnormal ovulation?

Doctor's answer:

Infertility due to anovulation, as one of the most prevalent conditions causing infertility today, has garnered increasing attention from women. However, many are still unaware that even after treatment, there is still a certain probability of recurrence.
Causes of anovulation:
(1) Turner syndrome and simple gonadal dysgenesis are chromosomal abnormal diseases, both primarily characterized by primary amenorrhea, elevated gonadotropins, and reduced estrogen levels. Pregnancy can be achieved through in vitro fertilization and embryo transfer using donor eggs.
(2) Premature ovarian failure and polycystic ovary syndrome (PCOS): High gonadotropin amenorrhea occurring before the age of 40 due to ovarian dysfunction accounts for approximately 20%–28% of primary amenorrhea and 4%–20% of secondary amenorrhea.
(3) Polycystic ovary syndrome (PCOS): Often or menstrual oligomenorrhea, long-term anovulation, excessive androgens, and non-cyclical estrogen fluctuations. It is currently a major cause of anovulatory infertility.
(4) Luteinized unruptured follicle syndrome (LUFS): In a normal menstrual cycle or medication-induced ovulation cycle, the ovary develops a dominant follicle, but the follicle does not disappear or persist in growth 48 hours after the LH peak, while the granulosa cells have undergone luteinization but the follicle remains unruptured.
Advice: It is recommended to undergo comprehensive etiological testing and actively treat the condition based on the identified cause. Wishing you good health!
