What is the mass in the left adnexal area?

Patient's question:

Ultrasound findings;
The uterus is anteverted, with a size of approximately 48mm x 41mm x 37mm, normal in shape,
homogeneous muscle layer echoes, a separated dark area with a width of 3.0 in the middle-lower part of the uterine cavity,
endometrial thickness of about 10.6mm.
No masses were observed in the right adnexal region. In the left adnexal region, a well-defined mass with a size of 42mm x 20mm was seen,
the inner wall was rough, with good transmissibility, showing septations, and the boundary with the left ovary was unclear.
The right ovary is 38mm x 30mm x 24mm in size, with multiple primary follicles inside.
