Does pregnancy require a glucose tolerance test?

Patient's question:

At 20 weeks of pregnancy, after fasting and drinking sugar water for one hour, the blood lipid level was measured at 7L. Is it necessary to undergo a glucose tolerance test? Is the glucose tolerance test the same as OGTT? Thank you.

Doctor's answer:

Hello: OGTT is the abbreviation for Oral Glucose Tolerance Test. In normal individuals, after consuming a large amount of glucose, blood glucose concentration does not exceed 8.9 millimoles per liter (160 milligrams per deciliter) and returns to normal levels within 2 hours. To facilitate early diagnosis of diabetes, the following individuals should undergo an Oral Glucose Tolerance Test (OGTT), but this test is prohibited for diabetic patients.
(1) Positive urine sugar, clinical suspicion of diabetes with questionable elevated fasting or random blood glucose levels.
(2) Follow-up for individuals with pre-diabetes (IGT).
(3) Confirmation of gestational diabetes in suspected cases.
(4) Differentiation of diabetes caused by other reasons, such as renal diabetes, trophic diabetes, stress-induced diabetes, etc., especially in cases with normal or questionably elevated fasting blood glucose.
(5) Family history of diabetes, normal fasting blood glucose but with symptoms or signs, with or without complications, particularly in patients under 50 years of age.
(6) History of spontaneous abortion, preterm delivery, stillbirth, or macrosomia during pregnancy, or non-pregnant adults with symptoms suggestive of hypoglycemia.
The steps for performing an OGTT are as follows: ① Three days before the test, follow a high-sugar diet, consuming approximately 300 grams of glucose-equivalent carbohydrates daily. ② Begin fasting after the evening meal the day before the test, with a fasting period of at least 8–10 hours, allowing for water intake. ③ Due to the diurnal fluctuations in blood glucose influenced by various physiological conditions, the test is best conducted between 7–9 a.m., with a 30-minute rest period before the test. ④ Draw 1.5
