Treatment methods for endometritis

Patient's question:

I had vaginal bleeding after sexual intercourse a few days ago. The ultrasound showed unclear endometrial lines, and the doctor diagnosed it as endometritis. Which medications would be better for me? I am currently taking Motherwort Capsules, Clarithromycin Dispersible Tablets, and Fumian Kangfu Capsules. Could you recommend any other medications that would be helpful? I am also expecting my period in the next couple of days.

Doctor's answer:

Hello, medication should be discontinued during the menstrual period. The drugs mentioned above can be used.
During endometritis, routine bacterial culture and drug sensitivity tests should be performed to select effective antibiotics. Before the test results are available or when testing is not feasible, broad-spectrum antibiotics such as penicillin can be used. After a negative skin test, penicillin (800,000–1,600,000 units) should be administered intramuscularly twice daily. If the bacterial culture is aerobic, gentamicin should be injected intramuscularly (80,000 units per dose, every 8 hours). If the culture results show anaerobic bacteria, metronidazole is more appropriate, taken orally (0.4 grams per dose, three times daily).
If acute endometritis occurs postpartum or after an abortion, it is important to consider whether there is still placental or fetal membrane residue in the uterine cavity. If residual tissue remains in the cavity, it should be removed after controlling the infection for 48–72 hours, followed by thorough uterine evacuation once the condition stabilizes. Postoperatively, oxytocin (10 units) should be injected intramuscularly, and oral preparations such as motherwort ointment or postpartum biochemical decoction should be taken to promote uterine contraction. Antibiotics should still be continued.
