Patient's question:
Hello!I am 31 weeks pregnant, and the ultrasound examination showed polyhydramnios. The results are as follows:
Biparietal diameter (BPD): 8.8 cm
Weight: approximately 1924 g
Head circumference (HC): 30.5 cm
Amniotic fluid index (AFI): 37.3 cm
Abdominal circumference (AC): 27.6 cm
Femur length (FL): 6.1 cm
No abnormalities were found in other areas, such as the lips, kidneys, neck, bladder, four-chamber view, spine, and limbs. The stomach bubble was not clearly visible. The ultrasound diagnosis was: "Unsatisfactory visualization of the stomach bubble, with possible upper gastrointestinal tract abnormalities in the fetus."
What should I do in this situation?
Doctor's answer:
Hello: The management of polyhydramnios primarily depends on whether the fetus has malformations and the severity of the pregnant woman's symptoms. If the fetus is without malformations and the symptoms are mild, pregnancy can continue. Rest, a low-sodium diet, and diuretics such as hydrochlorothiazide 25mg, taken orally three times daily, or traditional Chinese medicine that strengthens the spleen, promotes diuresis, and warms yang to transform qi may be recommended. In recent years, indomethacin (a prostaglandin synthase inhibitor) at a dose of 2.2-3.0 mg/kg/day has shown good therapeutic effects. If necessary, amniocentesis to release amniotic fluid may be performed to alleviate compression symptoms. If the fetus has malformations, pregnancy should be terminated.
