What causes bleeding after a cesarean section?

Patient's question:

Hello doctor, my wife had a previous placenta previa that resolved, and she underwent a cesarean section at 37 weeks. The operating doctor mentioned there was placenta adhesion. Six days after delivery, my wife's vaginal discharge stopped, but she still had abdominal pain. The doctor recommended using a hot water bag for relief. Now, on the 17th day, my wife's vaginal discharge has reappeared, resembling menstruation, but the amount is not significant. What could be the cause? What should we do?

Doctor's answer:

Postpartum hemorrhage after cesarean section should not last more than two weeks. Generally, bleeding stops within one week, followed by other types of lochia. If bleeding persists for over a month, it may be due to the impact of cesarean scar on uterine contraction, or residual tissue or secondary infection causing bleeding. There is no issue of uterine dislocation. You need to first identify the cause of bleeding before considering treatment. An ultrasound examination can be performed first.
