High blood sugar during pregnancy, do you need to inject insulin?

Patient's question:

Doctor, hello! I am currently 30 weeks pregnant and have detected high blood sugar at the hospital. Fasting blood sugar is 5.4, and after the 75g glucose tolerance test, it was 12.7 one hour later and 10.3 two hours later. Currently at home, I measure my blood sugar four times a day, with generally fasting levels of 5.2-5.8 and two-hour levels of 6.3-9.1. Please tell me if I need to inject insulin? Thank you for your reply!!

Doctor's answer:

If a pregnant woman has elevated blood sugar during the early stages of pregnancy, it usually indicates undiagnosed diabetes before pregnancy. Given that elevated blood sugar in early pregnancy can have a significant impact on the developing fetus, severe cases may lead to fetal malformations or even miscarriage. Therefore, once high blood sugar is detected, appropriate dietary control and exercise therapy should be initiated. If blood sugar levels remain uncontrolled after 3-5 days of dietary and exercise management, or if hunger-induced ketosis occurs after dietary control, and increasing food intake leads to hyperglycemia, insulin therapy must be started as soon as possible and continued until delivery.
