Why is there bleeding when the period hasn't arrived yet?

Patient's question:

Detailed medical condition and consultation purpose: Because I have been taking Marvelon (birth control pills), my periods have been very regular. I don't know why I had a small amount of bleeding this month while still on the medication, and it wasn't as much as a normal period. I have been taking the pills as prescribed, one a day, without missing any. Besides Pufengmin and Sanjiu Cold Relief, I haven't taken any other medications. Also, the bleeding started the day after my boyfriend left, and he is still away. In the first three days of the bleeding, we didn't do anything special. I want to ask, what could be the reason for this? Why would there be bleeding when everything else is normal?
Additional information: I was hiking on the day of the bleeding.

Doctor's answer:

Hello: According to your description, it may be ovulatory bleeding, or it could be related to overexertion.
During the mid-cycle, i.e., the ovulation period, a temporary decline in estrogen levels causes the endometrium to lose hormonal support, leading to partial shedding of the endometrium and resulting in regular vaginal bleeding, known as ovulatory bleeding. In traditional Chinese medicine, it is referred to as "intermenstrual bleeding."
Treatment
1. Western medicine treatment: Diethylstilbestrol 0.25–0.5 mg per day, starting 3 days before ovulation and stopping 2–3 days after the bleeding stops.
2. Traditional Chinese medicine treatment: Six-ingredient Rehmannia Pills, Solidifying the Menstrual Cycle Pills, and Blood Mansion Removing Stasis Pills.
Prevention and Care
1. Pay attention to menstrual health, maintain cleanliness of the external genitalia, and prevent pelvic infection.
2. Keep a calm mood and avoid excessive stress.
