What are the manifestations of varicocele?

Patient's question:

I have been feeling unwell recently. Sometimes my testicles have a heavy pain, but it doesn't hurt often. I went to the hospital for a check-up and was diagnosed with left varicocele. I am very worried and nervous.

Doctor's answer:

The symptoms of varicocele are mainly manifested in the following ways:
1. In the early stage, when standing, the varicose veins are not visible on the scrotal skin, but the dilated veins can be felt inside the scrotum. When lying down, the dilated veins disappear quickly.
2. Later, when standing, the dilated veins are visible on the scrotum, and the dilated veins inside the scrotum can be felt more distinctly. The symptoms of varicocele become more apparent.
3. In the later stage, the scrotal surface has obvious thick, large blood vessels, and the scrotum contains prominent, worm-like dilated veins with thickened and hardened vein walls. When lying down, the veins disappear slowly. Patients with severe scrotal varicocele, those with sperm counts below 20 million for three consecutive times, or those with testicular atrophy, as well as those whose dilated veins disappear when lying down, may undergo high ligation of the internal spermatic vein.
