What causes luteal dysfunction?

Patient's question:

I've been married for several years, and so far, we haven't had any children. I'm very anxious about it and don't know if it's my problem. I've asked my husband to get checked, but he refuses. I'm very worried that my condition might worsen. What could be the reason for luteal dysfunction?

Doctor's answer:

1. Insufficient hormone secretion during the follicular phase: Due to the imbalance in the secretion of follicle-stimulating hormone (FSH) and luteinizing hormone (LH), follicular development is impaired and luteal formation is defective, resulting in insufficient progesterone secretion by the corpus luteum after ovulation. The causes can involve various links of the hypothalamic-pituitary-ovarian axis and many factors that affect the function of this axis.
  2. Endometriosis and frequent miscarriages: Conditions such as endometriosis or frequent miscarriages can lead to an increased release of prostaglandins from the endometrium, thereby affecting luteal function. Even if the luteal function is normal, it still causes endometrial developmental disorders, leading to infertility. How is endometriosis treated in women?
  3. Insufficient LH secretion during the luteal phase: Delayed secretion changes or incomplete secretion during the luteal phase, insufficient progesterone receptors in the endometrium, and poor response of the endometrium to progesterone all contribute to this issue.
